After narrowly escaping the horrors of the Elite Hunting Club, Paxton grapples with PTSD and withdraws from society, seeking solace with his girlfriend, Stephanie. Their relationship becomes strained when Stephanie dismisses Paxton's paranoia as exaggerated and insufferable. The situation takes a gruesome turn when Stephanie wakes up to discover Paxton's decapitated body in their kitchen. The Elite Hunting Club's boss, Sasha Rassimov, receives a macabre package containing Paxton's severed head.
Meanwhile, in Rome, three American art students—Beth, Whitney, and Lorna—yield to the persuasion of Axelle, a nude model they are sketching, and decide to join her on a luxurious spa vacation in Slovakia. Little do they know, the hostel they check into becomes the gateway for their descent into the Elite Hunting Club's sinister world. The desk clerk secretly uploads their passport photos to an auction website operated by the club.
American businessman Todd and his friend Stuart win the bids for Whitney and Beth, embarking on an excited journey to Slovakia. Lorna, however, falls victim to a local named Roman during the village's harvest festival. She is kidnapped, stripped naked, bound, and subjected to a gruesome execution by a sadistic woman wielding a scythe.
Simultaneously, Beth, Whitney, Axelle, and a local man named Miroslav visit a spa to unwind. Beth wakes up alone, sans belongings, and becomes the target of pursuit. Fleeing from violent men, she is momentarily saved by Sasha and Axelle. However, Beth's ordeal continues as she discovers a room filled with human trophy heads, including Paxton's. Captured and taken to an abandoned factory, Beth finds herself in the company of Stuart, who struggles with his assigned task of killing her.
In another chilling scenario, Todd hesitates after accidentally injuring Whitney with a power saw. The Elite Hunting Club insists on Whitney's death, releasing attack dogs to enforce their command. Meanwhile, Stuart reluctantly accepts the club's offer to behead Whitney.
As the night unfolds, Stuart, now resolute, prepares to torture and kill Beth. Revealing a disturbing obsession, Stuart explains his deep-seated hatred for his wife, whom he has never had the chance to harm. However, Beth turns the tables by seducing Stuart, enabling her to escape and chain him to a chair. In a brutal act of vengeance, she cuts off Stuart's genitalia, leaving him to bleed to death.
Sasha, impressed by Beth's resolve, offers her a chance to become an official member of the Elite Hunting Club. Beth negotiates for her freedom with part of her inheritance, outbidding Stuart. Sasha, revealing Stuart's financial constraints, accepts Beth's offer. To seal her membership, Beth receives an Elite Hunting tattoo.
Later that night, Axelle falls prey to the Gypsy street children, and Beth takes her revenge by ambushing and beheading Axelle. The macabre scene continues as the children play football with Axelle's severed head. The dark and twisted tale reaches its unsettling climax, highlighting the horrors of the Elite Hunting Club's world.
