Plot:

The film opens on New Year's Eve 1999 in Bern, Switzerland. A young, arrogant Tony Stark is introduced to Aldrich Killian, a brilliant but socially awkward scientist who attempts to pitch his A.I.M. (Advanced Idea Mechanics) project to Tony. Tony dismisses him, promising to meet him on the rooftop later, a promise he never keeps. He then encounters Dr. Maya Hansen, a botanist who has developed Extremis, a groundbreaking regenerative treatment that can heal debilitating injuries but has dangerous, unstable side effects, including explosive overheating. Tony, ever the charmer, spends the night with Maya, inadvertently setting the stage for future conflicts.

Thirteen years later, following the events of "The Avengers," Tony Stark is suffering from severe anxiety attacks and PTSD, stemming from his near-death experience during the Battle of New York. He spends sleepless nights in his workshop, obsessively building and refining dozens of new Iron Man suits, neglecting Pepper Potts and his responsibilities. His reliance on the suits has become a coping mechanism, straining his relationship with Pepper, who now runs Stark Industries. Meanwhile, the world is terrorized by the Mandarin, a ruthless terrorist leader who broadcasts chilling messages and claims responsibility for a series of bombings, leaving behind no forensic evidence except for specific thermal signatures.

Tony's head of security, Happy Hogan, is severely injured in one of the Mandarin's bombings outside the TCL Chinese Theatre, where he was investigating a suspicious A.I.M. employee. Enraged, Tony publicly challenges the Mandarin, giving him his home address on live television. This reckless act leads to a devastating attack on his Malibu mansion by three attack helicopters, commanded by the Mandarin. Tony, still suffering from an anxiety attack, barely manages to get Pepper and Maya (who had come to warn him) to safety before the house is destroyed. He escapes in a prototype Mark XLII suit, which is still in its testing phase and prone to malfunctions. The suit, severely damaged, flies him according to a pre-programmed flight plan to Rose Hill, Tennessee, a seemingly random location linked to one of the Mandarin's unexplained bombings.

Stranded in rural Tennessee, with a malfunctioning suit and no resources, Tony is forced to rely on his intellect. He encounters Harley Keener, a resourceful and lonely ten-year-old boy who helps him repair the suit and provides a makeshift workshop. Together, they investigate the Rose Hill bombing, discovering that the explosions are not caused by conventional bombs, but by Extremis-infected individuals whose unstable powers cause them to explode. Tony realizes the thermal signatures left behind by the Mandarin's attacks match the unique heat generated by Extremis. He also discovers that the victims were all former soldiers who had been discharged due to disabilities, connecting the dots to Maya Hansen's earlier work.

Tony traces the Extremis technology back to A.I.M., now a prominent think tank led by Aldrich Killian, who has transformed from the awkward scientist of 1999 into a charismatic and powerful figure. Tony infiltrates a Mandarin compound in Miami, only to discover that the Mandarin is not a real person, but an actor named Trevor Slattery, a drug-addled English theatrical performer hired by Killian to be the public face of his terrorist campaign. Killian reveals his grand scheme: he used the Mandarin to create a climate of fear and chaos, developing Extremis as a "cure" for the injuries caused by his own "terrorist" attacks, thus creating a demand for his product and consolidating power. He also reveals that he injected Pepper with Extremis to force Tony's cooperation. Killian then captures Tony, intending to exploit his genius to stabilize the Extremis formula.

Colonel James "Rhodey" Rhodes, now operating as the Iron Patriot (formerly War Machine), is also captured by Killian's forces. Killian steals the Iron Patriot suit and uses it to kidnap President Ellis, planning to execute him on live television, blaming the "Mandarin" and framing Vice President Rodriguez (whose daughter is an Extremis patient being treated by A.I.M.) as a heroic figure who saves the day, thereby installing a puppet president. Tony, with the help of Harley, manages to escape Killian's captivity. He remotely calls in his entire Iron Legion – dozens of Iron Man suits he's been secretly building – to aid him in his fight against Killian.

The climax unfolds on an abandoned oil rig, where Killian intends to execute President Ellis. Tony and Rhodey, who also manages to escape, launch a full-scale assault on the rig, battling Killian's Extremis soldiers. The Iron Legion, activated by Tony, provides crucial support, engaging in a spectacular battle. During the chaos, Pepper, having survived the Extremis injection and now possessing its regenerative powers, falls into a fiery pit. Tony believes her to be dead, fueling his rage against Killian. In a brutal confrontation, Tony and Killian fight, with Killian proving incredibly difficult to kill due to his advanced Extremis regeneration.

Just as Killian is about to deliver a fatal blow to Tony, Pepper reappears, having survived the fall and fully manifesting her Extremis powers. She single-handedly dispatches Killian, using her newfound strength to tear him apart and then detonating a final explosion that incinerates him. With Killian defeated, Tony initiates the "Clean Slate Protocol," remotely destroying all of his Iron Man suits in a spectacular fireworks display, signifying his decision to move past his reliance on the armor. He undergoes surgery to remove the shrapnel near his heart, rendering the arc reactor unnecessary, and Pepper's Extremis is successfully cured by Maya Hansen before her death. The film concludes with Tony narrating that he is Iron Man, not because of the suit, but because of his ingenuity and resilience, having finally overcome his PTSD. In a post-credits scene, Tony is revealed to be recounting his story to Bruce Banner, who has fallen asleep.
