Plot:

The film opens with a haunting image of a young girl, Ofelia, bleeding from her nose, as a narrator explains the legend of Princess Moanna, who longed for the human world and eventually died, leaving behind a portal that would one day reopen. The story then shifts to 1944 Spain, where Ofelia and her heavily pregnant, ailing mother, Carmen, are traveling to a remote military outpost. Carmen is marrying Captain Vidal, a ruthless and cruel officer in Franco's fascist army, who is tasked with crushing the remaining Republican rebels, or Maquis, hiding in the surrounding forests.

Upon their arrival at Vidal's mill, which serves as his base, Ofelia immediately senses the oppressive atmosphere. Vidal shows little warmth towards his new stepdaughter, his primary concern being the birth of his son, whom he believes will carry on his legacy. The first task requires Ofelia to retrieve a key from the belly of a giant toad that lives under a fig tree. The tree is dying, choked by the toad's presence. She finds the repulsive toad and, using three magical stones given by the Faun, tricks it into regurgitating its internal organs, along with a golden key. She successfully retrieves the key but returns home covered in mud, further angering Vidal and distressing Carmen. The Faun, however, is pleased with her courage, though he warns her against future disobedience.

Meanwhile, the brutal reality of the war intensifies. Vidal relentlessly hunts the Maquis, torturing and executing them without mercy. Mercedes, the housekeeper, secretly provides food and supplies to her brother Pedro, a leader of the rebels, while Doctor Ferreiro, the camp physician, also secretly aids the Maquis. Carmen's pregnancy becomes increasingly difficult, and she is confined to bed, growing weaker. Ofelia tries to use the golden key from her first task, discovering it unlocks a secret compartment in Vidal's office where he keeps a map of the labyrinth, a small dagger, and a strange, chalk-drawn symbol.

For her second task, the Faun instructs Ofelia to use the key in a secret chamber beneath the mill, accessed by a piece of chalk that can draw magical doors. He warns her not to eat anything from the lavish banquet spread within the chamber. Ofelia enters a terrifying realm, a dining hall filled with tempting food, at the head of which sits the Pale Man, a monstrous, child-eating creature with eyes in his hands. Driven by hunger and curiosity, Ofelia disobeys the Faun's strict instructions and eats two grapes from the table. The Pale Man awakens, placing his eyes into his palms, and pursues her. In her desperate escape, two of her accompanying fairies are devoured. Ofelia manages to escape the chamber just in time, but her disobedience enrages the Faun. He declares her unworthy of returning to the Underworld and abandons her, stating that her mother will die, and she will never see her kingdom.

The consequences of Ofelia's disobedience are swift and tragic. Carmen's condition worsens, and she dies during childbirth, leaving behind Vidal's healthy infant son. Ofelia is heartbroken and feels utterly alone. The war outside grows increasingly violent. Vidal captures and brutally tortures a wounded rebel, but Mercedes, seeing her chance, attempts to escape with Ofelia. Vidal catches them, but Mercedes, revealing her true allegiance, stabs him repeatedly with a hidden knife and escapes, rejoining the Maquis. Doctor Ferreiro, confronted by Vidal about his aid to the rebels, defiantly asserts his humanity before being shot dead by the Captain.

With Carmen dead and the Faun seemingly gone, Ofelia is in despair. However, the Faun reappears, offering her one final chance at redemption. He explains that her brother, Vidal's infant son, is the key to her return to the Underworld. She must bring the baby to the labyrinth and offer a drop of his innocent blood to open the portal. The Faun explains that this is the final, most crucial test, requiring the sacrifice of an innocent. Ofelia, initially horrified, agrees, believing it is the only way to save herself and honor her true lineage. She drugs Vidal with a sedative and takes the baby.

Vidal, discovering his son is missing, pursues Ofelia into the labyrinth. He finds her at the center, holding the baby. The Faun stands by, urging Ofelia to complete the ritual. Ofelia, however, refuses to harm her baby brother, stating, "He is just a baby." The Faun, seeing her refusal to shed innocent blood, expresses his satisfaction. Vidal, unable to see the Faun, is bewildered by Ofelia's apparent conversation with thin air. He snatches his son from her arms and shoots Ofelia in the stomach. As Ofelia collapses, bleeding onto the stone altar in the center of the labyrinth, Vidal emerges to confront Mercedes and the approaching rebels. He hands his son to Mercedes, arrogantly telling her to tell his son the time of his death. Mercedes replies, "He won't even know your name," before Pedro shoots Vidal in the head, killing him instantly.

Mercedes finds Ofelia dying in the labyrinth. As Ofelia takes her last breath, her blood drips onto the altar. In a magical transition, Ofelia, now pristine and whole, appears in a golden throne room in the Underworld. Her true parents, the King and Queen, welcome her, explaining that her refusal to shed innocent blood was the final, true test, proving her purity and worth. She is Princess Moanna, and she has returned home.
